My most recent clinical setting experience was at a psych facility where I had my leadership clinical rotation. During my time at the facility, I had the opportunity to audit the charts. It looked like easy work, but it was not because they are still using the paper charting. The papers were not organized, not placed in the particular sections; all the sections were not completed by the staff due to short staff. They always have admissions and discharges, and the nurses have a lot of papers to fill up.
