Community diagnosis After careful evaluation of Sentinel City, I identified three major health challenges that are affecting the residents of this area, and these problems include but not limited to increased incidence and prevalence rates for sexually transmitted diseases, use of illicit drugs, and failure to embrace healthy behaviors such as disease vaccination. According to the healthy people 2020, diseases result from infectious viruses that are primarily transmitted through sexual intercourse. These viruses contribute to sexual infections such as human immunodeficiency syndrome, gonorrhea, Chlamydia, HPV, syphilis, hepatitis B, among others (ODPHP, 2016). In Sentinel City, the prevalence of Syphilis, Hepatitis B, and HIV have been high among adults (Individuals who are 25 years of age and above).
