During the clinic\’s leadership rotation this week, I surveyed 15 staffing PICOT questionnaire about my project title Understaffing of Nurses. Also, I did the audit of the closed charts. These charts belong to the patients who were no longer in the facility got discharged. This time we dig through the charts thoroughly and comparing with the ACADIA checklist to make sure everything is completed. It took a while to work on these charts. I was having difficulty to find the sections in the chart.

Introduction and Reflection on Action

My most recent experience in the clinical setting was during the clinic’s leadership rotation where I spent my time auditing the patient charts. I had to look through each chart and ensure that the information in them complied with the ACADIA | GET AN EXPERT FOR YOUR ASSIGNMENT | of the charts was unpleasant because it was a tedious, and time-consuming activity. It was also difficult to locate some of the sections on the chart, which complicated the process further. On the other hand, I enjoyed the activity because it helped educate me on how to audit charts efficiently.
